Title: Registered Nurse/ Manager
Position Summary:
The Clinical Staff Manager/Population Health Nurse performs a variety of essential administrative job duties include but not limited to scheduling, coverage arrangements, and supervision of the clinical staff, working with the Practice Manager related to development and implementation of clinical policies and procedures as well as clinical tasks directly related to providing and supporting quality patient care. Other essential job duties include working directly with providers to ensure all patients receive care according to the provider’s instructions; delivering excellent service by assisting patients over the phone and in person with any inquiries; and providing administrative support according to established Matagorda Medical Group protocol. This position is also responsible for promoting an effective team-based partnership between patients, families and providers to coordinate the care and health management of our patient population, managing the Annual Wellness Visit process for Medicare patients, overseeing transitional care management process and chronic care management visits, providing clinical health coaching, and promoting healthy lifestyle choices to mitigate future health risk.
Position Responsibilities:
‹ Serves as administrative authority for clinical staff by supervising and evaluating the clinical staff per established clinic protocol.
‹ Schedules and assigns clinical staff and ensures adequate coverage at all times
‹ Provides adequate orientation and professional development for new and existing clinical staff; determines acceptable timeframe to remediate gaps in necessary knowledge and skills.
‹ Offers useful development feedback for clinical staff; confronts known issues and develops plan with Practice Manager to address resolve performance issues. Counsels personnel as needed and completes appropriate documentation for employee files.
‹ Conducts routine surveillance of clinical staff to ensure quality patient care and physician support.
‹ Assists clinical staff in analyzing and resolving complex problems and implementation of new and established clinical policies and procedures.
‹ Oversee TVFC rules and regulations.
‹ Oversee clinical staff adheres to the RHC rules and regulations.
‹ Works with the Practice Manager as it relates to development and implementation of new and existing clinical policies and procedures as well as provides feedback for the continues improvement of the Population Health program.
‹ Orders and obtains needed medical supplies and equipment and ensures all items are dispersed accordingly.
‹ Uses the utmost tact and diplomacy while interacting with patients and staff to ensure excellent patient care and satisfaction. Exercises problem-solving and conflict resolution skills when handling complaints regarding clinical staff and refers to appropriate designated personnel as needed.
‹ Adheres to confidentiality and privacy guidelines in accordance with all Matagorda Medical Group policies and HIPAA requirements to protect patient’s personal and financial information.
‹ Maintains good relations with patients, staff, and other departments.
‹ Other assigned duties as the job requires.
Position Requirements:
‹ Active registered nurse license required; bachelor’s degree in nursing preferred.
‹ Three or more years of experience in clinical or community health settings required.
‹ Supervisor experience is required.
‹ Highly effective organizational skills; ability to create and maintain accurate records and assess and prioritize multiple request from patients, families, providers, staff and management team.
‹ Advanced working knowledge in insurance and Medicare guidelines, ACO, and MIPS quality reporting are highly preferred.
‹ Ability to fluently read, write, and speak in English required.
‹ Proficient in basic clerical skills and communication technology; Electronic Medical Record experience required.
‹ Ability and willingness to interact with all patients and coworkers in a professional and helpful manner.
Physical Demands:
‹ Prolonged, extensive, or considerable standing and walking. Lifting, positioning, pushing and /or transferring patients.
‹ Considerable reaching, stooping, bending, kneeling, and crouching.
‹ Lifting and/or moving items to 25 pounds.
‹ Regularly required to talk and hear.
